NHS Health CheckWe are now offering Health Checks to eligible patients which aim to lower your risk of diabetes, stroke, heart and kidney disease. The free 30 minute check will be available to those aged 40 – 74 years who do not already have a chronic disease.If you would like to be considered for this Check please leave your name at Reception. Patient Reference GroupHow Are We Doing?We are encouraging patients to give their views about how the Practice is doing. We would like to be able to find out the opinions of as many patients as possible and are asking if people would provide their email addresses so we can contact you by email every now and again to ask you a question or two. Mobile Phone InformationIf you have not already done so please would you inform us of your mobile phone number and give us consent to contact you via text. We will be using text messaging as a way of informing patients of health reviews that are due. We may also occasionally use text messaging to provide you with other Surgery information such as a due appointment. We will not use it for sensitive information such as test results and we not will issue any patient identifiable information. |
